Birmingham and Solihull ICS (BSol ICB) has a registered population of over 1.6 million people supported in primary care by 176 GP Practices/Practice Groups across five localities in Birmingham and one in Solihull. The key acute providers comprise: * University Hospitals Birmingham NHS Foundation Trust * Birmingham Womens and Childrens NHS Foundation Trust * The Royal Orthopaedic NHS Foundation Trust * Birmingham Community Healthcare NHS Foundation Trust * Birmingham and Solihull Mental NHS Foundation Trust * Sandwell and West Birmingham NHS Trust University Hospital Birmingham NHS Foundation Trust (UHB) is now one of the largest teaching hospital trusts in England treating more than 2.2 million patients each year from a regional, national, and international population. Further information can be obtained from https://www.uhb.nhs.uk/ The Trust is made up of the following; * Queen Elizabeth Hospital Birmingham (QEHB) * Birmingham Heartlands Hospital * Good Hope Hospital * Solihull Hospital * Solihull Community Services * Birmingham Chest Clinic * Along with a number of satellite units, allowing patients to be treated closer to home In addition, a range of multi-specialty independent sector providers operate under ICB contracts across the 6 localities. Currently, patient pathways across primary, community and secondary care are fragmented, with multiple manual processes for referrals, results handling, and patient communication. This creates inefficiencies, clinical variation, and a lack of patient visibility, all of which contribute to poorer outcomes and health inequalities. To address this, UHB is system lead on the procurement of a single, integrated digital pathway platform integrated within primary care to: * Deliver evidence-based care consistently across multiple specialities. * Improve efficiency and reduce the risk of error and unwarranted variation delivering care to the patient in the most appropriate setting, first time. * Facilitate patient choice and engagement, ensuring referrals are directed to the right clinic at the right time. * Improve quality of care, reduce health inequalities, and give patients visibility and involvement throughout their journey.

Renewal: The Contracting Authority proposes to enter into Contract(s) for a maximum period of 10 years with the successful Tenderer(s). The initial contract term shall be 4 years with the potential for six 12 month Contract extension periods by election of the Contracting Authority, on such notice and for such periods as is specified by the Contract. The Initial Term will be subject to a review period prior to the commencement of Year 3.
