NHS Arden and Greater East Midlands CSU on behalf of NHS Coventry and Warwickshire ICB are seeking to run a market engagement event for providers who have the relevant experience and skills to provide a Specialist Community Gynaecology Service (SCGS).

The aim of the contract is to provide a Specialist Community Gynaecology service with care as provided by CQC registered providers that are able to meet the health care needs of patients.

The Specialist Community Gynaecology Service (SCGS) will deliver a specialist primary care Gynaecology service for a range of conditions which do not require hospital care, enabling access to assessment, diagnosis, treatment and support for patients in the most appropriate setting, within the community. It is a transformative, integrated delivery model across primary and secondary care, with the overarching aim to support reducing the volume of referrals to secondary care gynaecology services when an enhanced primary care solution can be provided.

The objectives of this project are to commission a SCGS that delivers high-quality care, assessment, diagnostics and treatment for Coventry and Warwickshire patients with gynaecology conditions.

The Specialist Community Gynaecology Service will:
* Provide a simple referral process for Coventry and Warwickshire GPs identifiable via NHSE e-Referral Service (eRS), ensuring ongoing accuracy of eRS Directory of Services (DOS).
* Develop agreed referral pathways between SCGS and secondary care providers.
* Clinically triage all referrals for SCGS, moving towards gatekeeper for all system gynaecology referrals submitted from general practice via eRS, ensuring clinical appropriateness based on clinical symptoms as described in the referral documentation.
* In line with national guidance, aim to assess, diagnose and treat service users from 1st contact appointment, delivering follow up appointments and/or patient initiated follow up pathway (PIFU) based on clinical need.
* Provide specialist advice and guidance to referring clinicians via eRS, to support the management of patient care.
* Manage all required diagnostics on behalf of the service users registered GPs, reducing delays to patient care by developing agreed referral pathways with diagnostic providers.
* Provide all service users with choice of appointment options dependant on patient and clinical need; in person, virtual or group clinics.
* Implement hub and spoke community clinic models in each place, ensuring equity in access across Coventry and Warwickshire accessible via public transport.
* Optimise outpatient and treatment service appointments through robust planning of clinics, minimising DNA rates and ensuring delivery against 18 weeks RTT.
* Support service users to achieve self-management of condition/s, preventing re-referral to the service where appropriate.
* Work closely with primary care and secondary care clinicians to ensure optimal care is provided for patients, supporting the system wide gynaecology service offer for patients.
* Ensure all diagnosis points and necessary treatments are provided in line with national standards and best practice.
* Support the development of the system's primary care workforce through the provision of training, shadowing or clinical supervision, supporting the attainment of qualifications or as part of CPD.
* Work collaboratively across the system, developing effective relationships with stakeholders including primary care, secondary care gynaecology consultants and operational teams, system women's health leads and the Integrated Sexual Health (ISH) service provider.
* Ensure the service workforce maintain and strengthen specialist knowledge in women's health and gynaecology conditions, as part of CPD.
* Report service activity in line with locally agreed protocols.

Service description/care pathway to include:

The service will be delivered at each Place by expert GPs with extended roles (GPwERs) and nursing staff specialised in gynaecological conditions and wider women's health. The service provider will collaborate across the system to ensure optimisation of improving health outcomes for patients.

The provider will manage all referrals into the service including triage, to determine the appropriate service pathway. Where the referral meets the eligibility criteria for SCGS, the service provider is responsible for managing the presenting condition from assessment, undertaking required diagnostics and pathology testing, the provision of treatment required and providing appropriate advice and guidance for patient and referring GP. Service user outcomes will be shared with the referring GP at the end of any pathway including diagnostic and pathology testing outcomes.

During the lifetime of the contract, it is the system's intention for SCGS to triage all primary care gynaecology referrals sent to secondary care services. This will ensure clinical appropriacy, implementation of principles of referral management optimisation and identification of training opportunities for the primary care workforce.

The service provider will be accountable for clinical delivery, supervision and leadership and will appoint a GPwER Clinical Lead. The Clinical lead will be responsible for the delivery of the service as outlined in this specification. The Clinical lead will be available and contactable when necessary, retaining overall responsibility during service operating hours.

The intention of the ICB is to put in place a 3 year contract with the option to extend for up to a maximum of 2 years as a result of any process run under the Health Care Services (Provider Selection Regime) Regulations 2023 following this market engagement event. The anticipated contract value is c.PS0.45m/annum, so the anticipated current total contract value over the full life of the contract (5 years if the extension were to be enacted) would be c.PS2.25m. Please note that this is an indicative value for the purposes of this market engagement only and the ICB reserves the right to vary this value in any subsequent notice they may place.
