                    "rationale": "Need for additional works, services or supplies by the original contractor/concessionaire: The current service delivery model is inconsistent for patients and therefore there is an opportunity to commission the service differently by expanding the existing community/ Palliative Care in Partnership (PCiP) Service run by Mid Cheshire Hospitals NHS Foundation Trust to enable more co-ordinated / sustainable provision of care and better use of resources. The funding allocated to Mid Cheshire Hospitals NHS Foundation Trust is to supplement and develop the existing service. The original contract value was PS370,545,000 and the modification value is PS3,100,000. Therefore the total contract value will be PS373,645,000."
