Lot Information
Low Tech Homecare Services
This Lot covers the dispensing and delivery of medications to the patient's home, for them to self-administer after training and assessment by a healthcare professional. This service allows patients to manage their own treatments, promoting independence and reducing the burden on hospitals by enabling them to receive care in their own communities.
Mid Tech Homecare ServicesThis Lot covers the dispensing and delivery of medicines, often requiring significant clinical support, such as nurse training for self-administration or active monitoring for complex therapies like infusions, blood monitoring, or unlicensed products. These services will allow patients to receive treatment in a community setting while ensuring they have the necessary monitoring and support for safe and effective self-management of their conditions.
High Tech Homecare ServicesThis Lot covers the dispensing and delivery of complex and specialised medicines that require direct administration by qualified personal for complex treatments that cannot be safely managed by the patient alone, even with training. High tech activities will include, but not limited to: * Intravenous infusion * Self-administration needing advanced aseptic technique and / or portable equipment /specialist ancillaries * Products that are compounded aseptic medicines * Administration by healthcare professional * Provision of refrigeration equipment * Clinical decisions taken within approved clinical protocol with escalation of out-of-limit findings to the clinically responsible person * Chemotherapy compounding * Cancer Care * Palliative Care
Remote Patient Monitoring and Virtual WardsRemote Patient Monitoring and virtual wards allow patients to receive hospital-level care and treatment in their own homes or usual place of residence, using digital technology to monitor their condition remotely. Remote Patient Monitoring (RPM) is where healthcare providers monitor patients outside the traditional care setting using digital medical devices, such as weight scales, blood pressure monitors, pulse oximeters, and blood glucose meters. A Virtual Ward, also known as Hospital at Home, is a model of care that provides hospital-level treatment to patients in their own homes or care homes, rather than in a hospital. They are staffed by multidisciplinary teams (MDT) using technology for remote monitoring and communication.
