GPs to deliver community-based NHS Health Checks to improve the number of people over 40 accessing preventive assessment and advice; improve equity, ensuring that those who are most at risk are identified and prioritised appropriately; and to optimise opportunities to improve health and address risk factors for cardiovascular disease.

GPs to deliver community-based NHS Health Checks, which is aimed at providing healthy adults aged 40 to 74 with checks ups that look for early signs of stroke, kidney disease, heart disease, type 2 diabetes, or dementia. An award has been made following direct award Process B. The Lifetime Value of the award is PS1,875,000 and the contract will provide the above services from 1st April 2026 until 31st March 2031. This is a new service with new providers.
