Provision of a Key Worker Service to support children and young people (0-25) with learning disabilities and/or autism, delivering coordinated, person-centred, multi-agency support aligned to NHS England Keyworker Programme requirements.

The West Yorkshire Keyworker Service (WYKWS) supports children and young people aged 0-25 with autism and/or a learning disability who have complex needs and are at risk of mental health inpatient admission.
The service plays a key preventative and system-coordination role across health, education, social care and the voluntary sector, operating across Bradford, Calderdale, Kirklees, Leeds and Wakefield.
The service will provide:
Dedicated Key Workers delivering:
Assessment, engagement, care coordination, advocacy and ongoing case management
Multi-agency working across:
NHS services, Local Authorities, education and VCSE partners
Support for:
High-risk and complex cases, including safeguarding-sensitive situations
Participation in:
Dynamic Support Registers and Care, Education and Treatment Reviews
The service aims to:
Prevent hospital admissions and escalation
Improve coordination of care across systems
Enhance outcomes and experience for children, young people and families.
