Notice Description

This contract is to deliver support for an individual requiring placement in a care home. Location is an important factor in this contract, fulfilling the individual's wishes. Support provided covers personal care, meal preparation, and supporting all aspects of an individual's day to day living. The placement will also cover their living arrangements and ensure that full support is provided to meet the requirements of the person being placed. The service will include, as a minimum for each resident, the following facilities: Sole use of own bedroom with en-suite bathroom facility/ shared bathroom, with appropriate bed, armchair and storage Access to dayrooms/communal areas e.g. dining room, lounge, gardens etc Lighting and heating Laundry All necessary personal care Meal and drink provision that meets the needs of each individual resident Medication This contract is a continuous contract with ongoing rolling terms. The contract will terminate when all individual placements made through this contract are ended (there are known as the Individual Service Agreements).
