Summary of the contracting process

NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board commissioned routine elective secondary-care services for eligible patients using patient-choice arrangements. Somerset Surgical Services provides outpatient, diagnostic and treatment pathways across general surgery, including endoscopy and upper gastrointestinal procedures; orthopaedics and musculoskeletal care, including joint and pain management; ophthalmology; oral and maxillofacial surgery; dermatology; and urology. Some overlapping spinal treatment is included where the ICB is the commissioner. Ear, nose and throat and gynaecology services may also be provided within the contract scope. The provider is responsible for the patient pathway, from referral management and triage through diagnostics, consultation, assessment, treatment, discharge and follow-up. Delivery is from Weston General Hospital in Weston-super-Mare, Somerset; other premises require the commissioner’s written approval.

The procurement is complete and was awarded to Somerset Surgical Services under the Provider Selection Regime’s Direct Award Process B, following Right to Choose requirements; it was not a competition open to bids. The award used one criterion: patient choice, weighted at 100%, with price weighted at 0%. One bid was recorded. The contract period is 1 April 2026 to 31 March 2027, with no extension provision. The contract was signed on 1 April 2026. Its published contract value is £0.01, but the service is activity-based and has no guaranteed value, so this figure should not be read as a guaranteed payment or overall service budget. The contract has been formally awarded and is recorded as active.

For providers of elective secondary-care services, this award shows demand for a broad, coordinated surgical and outpatient offer serving patients through choice arrangements. A credible competitor would need to deliver the full pathway, including referral handling, triage, diagnostics, consultations, pre-treatment assessment, treatment, discharge and follow-up, across several specialties. Relevant capabilities include general surgery and endoscopy, orthopaedic and musculoskeletal care, ophthalmology, oral and maxillofacial surgery, dermatology and urology; the scope also allows ENT and gynaecology. Delivery depends on suitable hospital premises in the area, with commissioner approval needed for additional sites. The award was made through a direct selection process focused entirely on patient choice rather than price, indicating that a future provider would need to demonstrate how its service supports that choice and meets the commissioned pathways.
