Summary of the contracting process

NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board commissioned routine elective secondary-care shoulder surgery for patients. The service covers the care pathway where appropriate: referral, triage and clinical assessment; diagnostics, consultation and pre-treatment preparation; outpatient, ambulatory or inpatient treatment; discharge planning; and specialist follow-up or onward referral and self-care support for up to 14 days after discharge. The commissioning was conducted with Gloucestershire ICB as part of a formal clustering arrangement ahead of the two organisations’ planned merger. The work is healthcare services, specifically surgical hospital services, rather than a supply of goods or construction. The delivery area is the Bristol, North Somerset and South Gloucestershire area. NHS South, Central and West Commissioning Support Unit acted on behalf of the commissioners in issuing the procurement.

The procurement is complete and the contract was awarded to Severn Hospital Ltd through Provider Selection Regime Direct Award Process B, using a direct award without a prior competition. The published outcome records one bid. The award is active; the contract is active and was signed on 1 June 2026. The award period runs from 1 July 2026 to 31 March 2027, with no extension provision. This is an activity-based contract with no guaranteed value; the contract value is recorded as £0.01. The award criteria were patient choice, weighted at 100%, and price, weighted at 0%. The procedure was conducted under the Health Care Services (Provider Selection Regime) Regulations 2023, and the commissioners state that the Right to Choose requirements were followed.

This award shows demand from the Bristol, North Somerset and South Gloucestershire commissioners for routine elective shoulder surgery delivered across a complete secondary-care pathway, including assessment, treatment, discharge and short-term follow-up. Suppliers seeking to compete for a comparable requirement would need to demonstrate the ability to deliver appropriate outpatient, ambulatory or inpatient surgery and to coordinate referral, diagnostics, clinical assessment and post-discharge support. The evaluation placed the entire weight on patient choice, with no weighting for price, so a credible provider would need to show how its service supports patients’ choices and meets the commissioners’ service requirements. The contract was awarded through the Provider Selection Regime’s Direct Award Process B and followed Right to Choose requirements; familiarity with those arrangements and the ability to provide the required activity-based service would be relevant.
