Summary of the contracting process

Hull University Teaching Hospitals NHS Trust intends to commission primary care respiratory services to identify and treat people with chronic obstructive pulmonary disease (COPD) in Hull and East Yorkshire. The project will establish and evaluate a specialist-led neighbourhood care model, using digital risk stratification and multidisciplinary specialist review to find people at greatest risk, support prevention and improve access to specialist care in community settings. Primary Care Networks (PCNs) will work with respiratory specialists based at Hull University Teaching Hospital to identify, review and manage patients and strengthen primary care capability. Services will be delivered at PCN sites across the Hull and East Yorkshire area. The project is intended to test the model’s feasibility and impact and inform potential future expansion across the Humber region and beyond.

This is at the planning stage: the buyer has set out an intention to procure, but has not issued an invitation to tender. The planned route is the Most Suitable Provider process under the Health Care Services (Provider Selection Regime) Regulations 2023. The buyer expects an initial 12-month contract running from November 2026 to November 2027. The stated estimate is £7,400 for each appointed PCN, with three PCNs envisaged and a stated total of £22,200. The planning information also gives 2 November 2026 as the expected deadline for compliant expressions of interest and 6 November 2026 as the expected date for a further notice. The planned selection is for PCNs or primary care organisations meeting the stated population, infrastructure and respiratory-service requirements; no separate evaluation criteria are described.

The work is aimed at Primary Care Networks or other primary care organisations serving at least 30,000 registered patients. Suitable providers need the scale, infrastructure and capability to deliver high-quality, sustainable respiratory services, particularly in communities with a significant burden of respiratory disease. The buyer specifies a recorded QOF COPD prevalence above 2.5% in the population served and experience meeting the needs of such communities. Providers should be equipped to collaborate with specialist respiratory teams, use digital risk stratification and multidisciplinary review to identify high-risk patients, and support their ongoing management and preventive care in community settings. The project also involves implementation and evaluation, so organisations able to contribute to testing the neighbourhood model and building primary care capability are well placed to prepare. The planned collaboration offers a role in generating evidence to inform future commissioning and possible wider adoption.
