Forest Walk Uphall West Lothian - Management and Care Service
Notice Description
West Lothian Council own a Unit at Forrest Walk, Uphall which is used to provide care and support for physically disabled service users with complex needs who have been assessed as requiring 24 hour support within a registered care home environment in the community. This Unit requires on-site 24 hour staffing. It is likely that the residents will also have a high level of cognitive or intellectual impairment. The Authority are looking for a service provider to provide a management and care support service. The unit provides the following: 1. Residential Provision - four en-suite bedrooms and living area for long-term placements ; 2. Respite Provision - two en-suite bedrooms and living area for respite care placements ; 3. Day Care Provision - two places per day ; 4. Emergency call - overnight service for adapted tenancies.

Lot 1
A service provider will be expected to manage the Unit at Forest Walk, Uphall. The Unit a provides the following: Residential Provision - four en-suite bedrooms and living area for long-term placements ; Respite Provision - two en-suite bedrooms and living area for respite care placements ; Day Care Provision - two places per day ; Emergency call - overnight service for adapted tenancies. The objectives of the service include but need not be limited to : enabling a service user to live within an environment that respects the independence, dignity and respect of the service user ; providing care and support to the service user to ensure they are valued as a citizen in the life of the community and to develop friendships and relationships ; ensuring the service user has the opportunity and appropriate support to make informed choices in all aspects of their life ; developing a range of activities and support methods for the service user within the care home and the community ; supporting the development of skills that will result in an ordinary and positive lifestyle for the service user ; developing and offering a range of activities specific to each service user that takes account of the service user as a unique individual ; developing person centred planning and flexible responses to allow the service to be developed with the service user central to the service delivery. West Lothian Council developed the purpose-built Unit for people with physical and complex disabilities at 132, Forrest Walk, Uphall, EH52 5PW and are the owners of the development. The Council will be responsible for maintenance to the fabric of the property. This includes cyclical maintenance and reactive repairs, including repairs following graffiti or acts of vandalism. The on-going provision and delivery of management and care services within the Unit will be the responsibility of the provider. The provider will be responsible for delivering the service in a manner which has due regard for the upkeep of the fabric of the property. The provider will be responsible for the day to day cleanliness and maintenance of the fabric of the property and will undertake those duties and tasks associated with compliance with premises related health and safety. The successful tenderer will be required to enter into a lease of the premises in addition to entering into a service contract. The lease start date, end date and lease length will mirror the contract terms. The rental value of the property is 29,700GBP per annum and all tenderers are required to include provision for paying this rent in their tender return. Under the terms of the lease, the council will be responsible for maintemance, repair and statutory compliance of the property and it's contents. For greater clarity and for your purposes, a a schedule of responsibility for property and facilities costs will be included within the tender.
Renewal: The Authority may extend the contract for a further period (s) of up to 24 months in total. However, the contract will not exceed 31 March 2023.
