At its core this service should work to enable at risk individuals to sustain their tenancies, become active members of their community, and develop their capacity to live independently. These organisations have an established track record of delivering partnership programmes across their communities. By acting as a catalyst for positive change for the most vulnerable within these communities, the service aims to provide a relationship-led, person-centred approach, offering tailored support and advice to individuals with complex support needs. At its core this service should work to enable at risk individuals to sustain their tenancies, and develop their capacity to live independently. The project will work with individuals, whose issues include a combination of the following: - Mental ill health with extreme vulnerability - Substance misuse with chaotic lifestyle - Lack of independent living skills - Physical or Learning Disability - Serious Anti-social behaviour - Serious arrears - Poor physical and emotional health - Breakdown of family; including bereavement, disputes and abuse
