Provision of an outcomes focused housing support service, which will be delivered to people aged sixteen years and over who are homeless or at the risk of being homeless.

Outreach Housing Support Service - Buchan
The service will deliver an outcomes focused Outreach Housing Support Service for clients who are homeless or at the risk of being homeless. Individuals referred to this service will come from a range of backgrounds with varying levels of complexity depending on where they are in their life's journey; and may also have a range of health needs e.g. mental health, learning disabilities, physical disability, substance misuse etc. The purpose of the support is to enable clients to make the successful transition to independent living and tenancy sustainment provided from a range of accommodation, including, but not limited to, supported, temporary, shared, private/secure tenancies and family homes. The support provided will be tailored to meet the needs of each client, providing those with the necessary life skills to make the most out of their lives giving them a better quality of life and to support them meet their full potential. The support will be flexible to meet the clients' changing needs, with a key aim to enable clients' to sustain and maintain their tenancies. Provision of a housing support service for clients' needs and inputs will vary as they progress through their individual journeys. Currently, approximately 139 hours per week of housing support is commissioned from the independent care and support sector in Aberdeenshire. The approximate hours are given as a guide only and is not a guarantee of hours. The contract will be a single supplier framework agreement and all call off orders will be spot purchased. Main Settlements,Auchnagatt,Fetterangus,New Leeds, St Fergus,Boddam,Hatton,New Pitsligo,Strichen,Crimond, Longside,OldDeer,Stuartfield,Clola,Maud,Peterhead, Longhaven,Cruden Bay,Mintlaw,St Combs and Knaven. The townships in the table are given as a guide only. Clients may live out with towns and villages in rural Buchan.. The maximum hourly rate is GBP 18.05 The maximum social mileage rate per mile is GBP 0.45 Note this is not a social work service. This service is being commissioned by Aberdeenshire Council Housing - Homeless Service.

Individuals referred to this service will come from a range of backgrounds with varying levels of complexity depending on where they are in their life's journey; and may also have a range of health needs e.g. mental health, learning disabilities, physical disability, substance misuse etc. The purpose of the support is to enable clients to make the successful transition to independent living and tenancy sustainment provided from a range of accommodation, including, but not limited to, supported, temporary, shared, private/secure tenancies and family homes.
