The principal aim is that independent intermediate care will be provided for service users whose medical condition has stabilised in hospital, but for whom rehabilitative care is deemed necessary to enable the service user to achieve an increased level of functioning that will enable them to live safely (with support as necessary) at home or for service users for whom a period of rehabilitative care within a care home setting may prevent admission to hospital.
