The contract will provide flexible community care and support provision to 5 adults with Autism Spectrum Disorder / Learning Disabilities and associated complexities residing within West Lothian in a Core and Cluster living environment. Ongoing flexible care and support will be required 24 hours a day, 7 days a week, to assist the service users to achieve their assessed personalised outcomes and maximise their potential. Although the contract focuses on the 'Core' component of the Service in the first instance (which will be situated in a block of 4 adjoining 2 bedroom new-build flats), the proposed model forms part of a re-design of Learning Disability Services in the West Lothian locality and the 'cluster' element will be developed at a later date by WLHSCP whereby care and support will be offered to residents in geographical proximity to the Core. The estimated budget of the Core Service is 200,000 GBP per annum.

Renewal: The contract may be extended for a period of up to a further two years
