Notice Description

Dundee City Council is seeking Service Providers with the appropriate experience to provide a care at home service to adults and older people under a block contract arrangement on behalf of Dundee Health and Social Care Partnership. The intention is to have a range of Service Providers on a range of block contract arrangements from 500 to 1,500 hours per week.

Lot Information

Block Contract for 500 Hours per Week

Providing a personalised care at home service to adults and older people to meet the individual's agreed outcomes (500 hours per week block contract arrangement). The support may include personal care, social care/emotional support, assistance with health care and food and nutrition.

Options: Additional care at home service provision can be purchased under the same contractual arrangements.

Block Contract for 750 Hours per Week

Providing a personalised care at home service to adults and older people to meet the individual's agreed outcomes (750 hours per week block contract arrangement). The support may include personal care, social care/emotional support, assistance with health care and food and nutrition.

Options: Additional care at home service provision can be purchased under the same contractual arrangements.

Block Contract for 1,250 Hours per Week

Providing a personalised care at home service to adults and older people to meet the individual's agreed outcomes (1,250 hours per week block contract arrangement). The support may include personal care, social care/emotional support, assistance with health care and food and nutrition.
