Glasgow City Council invites tender bids for short-term care and support service for adults, who are being discharged from hospital, following a period of assessment and treatment due to mental ill health. The contract is expected to run for one year, with the option for Glasgow City Council to extend by a further one year. This requirement is not divided into lots due to its size and value. This procurement will be conducted via the Public Contracts Scotland - Tender portal (PCS-T). We will apply a single stage Open procedure. Bidders must self-certify their adherence to the conditions of participation via the SPD in PSC-T, and may be required to submit Means of Proof before contract award. Bidders must refer to the specific requirements listed in Section III in this Contract Notice when completing the SPD in PCS-T.

Budget is 150,000 GBP per annum- One year initial contract period with option for the Council to renew for a further one year period, subject to continued funding and service review to establish the success of service delivery and outcomes achieved. The Mental Health Discharge Support service will support Glasgow City Health and Social Care Partnership (GCHSCP) to respond effectively and improve outcomes for adults, with severe and enduring mental health problems, who are being discharged from hospital. This service will deliver outcomes for individuals in line with the Five-Year Strategy for Adult Mental Health services in Greater Glasgow and Clyde 2018-2023 and the priorities set out in the GCHSCP draft Strategic Plan 2023-2028. The draft HSCP Strategic Plan highlights the need to improve the overall health and wellbeing and prevent ill-health of the people of Glasgow. Prioritising prevention, early intervention and harm reduction models are they key drivers to achieve this aim. This service aligns with the Scottish Government's Mental Health Strategy 2017-2027, where the focus is on actions to offer prevention and early intervention, accessible services and physical wellbeing for people with mental ill health. The service will be delivered across the 3 Glasgow City HSCP localities and offer- - Services to support a successful and timely discharge of people from hospital to a community setting - Support the transition to community living, by providing immediate social supports on discharge from hospital - Services to the supported person to manage their mental health and any relapses they experience during the period of support. Please note: Economic operators may be excluded from this competition if they are in any of the situations referred to in regulation 8 and 9 of the Procurement (Scotland) Regulations 2016. Please note that any tender submission which exceeds the defined maximum budget of 150,000 GBP per annum will be disqualified

Renewal: One year initial contract with option for the Council to renew for further one year, subject to service review to determine the success of service delivery and outcomes achieved and available funding.
