Forth Valley College is undertaking a procurement exercise to appoint a suitably qualified and competent Contractor for the provision of Occupational Health related services for Staff. The Contractor is expected to demonstrate that their location is suitable for access for Forth Valley College Staff to travel to. Services required include Statutory Health Surveillance clinics and appointments, Attendance Management Advice, Individual Stress Risk Assessments and Pre-Employment Health Screening Questionnaires.. The below documentation requires to be submitted as part of the tender response: Single Procurement Document (SPD) Appendix A - Form of Tender Appendix B - Freedom of Information (if applicable) Appendix D - Sustain Supply Chain Code of Conduct Appendix E - Pricing Schedule Appendix F - SCM RP: Section 1 Appendix I - Quality Questionnaire

Renewal: The Authority reserves the right to extend the Agreement for a further 3 periods of 12 months subject to satisfactory performance and continued Authority requirements.
