The Community Link Worker Service will support people aged 16 and over to manage their health and wellbeing by addressing non-medical social factors. Delivered under the General Medical Services (GMS) contract in Scotland, Community Link Workers work alongside GPs and primary care teams as non-clinical practitioners, providing holistic, person-centred support to individuals experiencing issues such as social isolation, mental wellbeing concerns, financial hardship, housing difficulties, bereavement, loneliness, and other social factors impacting health and wellbeing. Through one-to-one support, social prescribing, and supported access to local services and community organisations, Community Link Workers help individuals identify personal goals, improve resilience, and engage with appropriate community-based support. The service also strengthens links between GP practices and community resources, supporting earlier intervention, reducing health inequalities, and improving overall wellbeing through non-medical approaches that complement primary care services.
