Provision of Intermediate Care and Reablement Services
Description: Medway Council are reprocuring an Intermediate Care and Reablement Service. The service will include home-based intermediate care and reablement for Pathway 1 acute hospital discharges and Bed-based intermediate care and reablement for Pathway 2 acute hospital discharges as well as more complex care such as bariatric and non-weight bearing pathways, as one service. This is a change from the current service contract, which was procured in two separate lots, one for each pathway described above. In delivering these services, the Service Provider is required to be fully compliant with the Care Quality Commissions (CQC) guidance, policies and protocols and thereafter comply with all statutory requirements as detailed in this specification.
