Notice Description

Hampshire County Council invite suitably experienced providers to tender for an Extra Care Service in Winchester, Hampshire. This service, delivered in a central location in Winchester, is for the provision of on-site 24 hour, 7 day a week social care services within a new Extra-Care housing scheme to improve choice and outcomes for vulnerable older and/or disabled people who are at risk of increased dependence without such provision. Within this service model providers will be expected to work in partnership with Adult Services, landlords, stakeholders and other partners within the local area to maximise the health and wellbeing of all service users on a long term basis, to meet the diverse needs of people within the Extra-Care scheme (including those with dementia) - including through the use of innovative solutions that improve choice, maximise the use of resources, avoids duplication and reduces bureaucracy. Service providers will have a responsibility for the wellbeing of residents 24 hours a day, seven days a week, providing non-institutional, personalised services and delivering planned care and unplanned care both during the day and at night to ensure people do not need to move to alternative higher level statutory services such as residential care, nursing care or hospital admissions through choosing to remain living in their home. Service providers will be required to maximise a person's health, wellbeing and independence, taking into account all the elements that are important to this; (these elements include use of assistive technology, aids and adaptations as well as those elements that maintain the dignity and self-respect of the individual such as the skills, motivation and confidence to care for oneself and live a fulfilling and meaningful life). This service aims to ensure that people are supported to live lives that are meaningful to them and to maximise opportunity for individuals' active participation within their local community, and ensure that they do not instead become passive recipients of services. The contract term is for a maximum of 4 years, the anticipated contract commencement date is October 2016, with an option to extend for a further 3 years (1+1+1) with a review clause after 18 months. Interested parties should register their interest in the project via the In-Tend website https://in-tendhost.co.uk/hampshire, and complete and submit the tender return document together with any supporting information requested by 14.00hrs on Tuesday 11th April 2017.
