17-118 Carers Support Worker Service (Dementia) and Dementia Cafe
Notice Description
The current contract for the Carers Support Worker Service (Dementia) and Dementia Cafes will expire in March 2018. The service specified offers a combination of group and individual support to: * Carers caring for people with all levels of dementia and * Individuals experiencing low to moderate levels of dementia. Group support will be delivered to both of the above client groups via a number of Dementia Cafes across the Rotherham Borough. Individual support will be offered in people's homes via telephone contact and at various locations mutually agreed with the service user within Rotherham.
