Notice Description

Funding from the Improved Better Care Fund and Public Health has been made available for a two-year pilot in Great Yarmouth. Partners from across Health, Social Care and the Borough Council have agreed a proposal which will see 'Living Well Connectors' engaging with local residents, finding out what matters to them and 'co-producing' a plan to link them with appropriate support as an alternative to, or to complement medical intervention. They will enable and support residents to take up activities within the community which build on their strengths to address needs and meet aspirations. The Council are seeking to commission an organisation to directly employ the Living Well Connectors and provide the services detailed in the specification for an initial period of 12 months with an option to extend for a further period of 12 months. This is an open tender, but the Council anticipate that bidders will be from the voluntary, community and social enterprise sector.
