The overarching purpose/output of this project is to develop a strategy for personalisation for Tower Hamlets for the next 3-5 years. (Pro Contract Link: https://procontract.due-north.com/Login/) This strategy will need to include and/or be informed by: 1. A mapping/consolidation exercise of work that has happened to date 2. A quantitative and qualitative evaluation of the work that has happened to date 3. A vision for personalisation for Tower Hamlets, which has been developed and agreed with all key partners in the system 4. Priority areas of focus for future years, working towards the vision that has been agreed 5. Details on the level of resource that may be required to deliver the strategy going forward, for example, in terms of programme management costs 6. Details on how PHBs can be funded on a sustainable basis going forward, for example, through extracting money from existing contracts 7. A delivery plan, to accompany the strategic objectives 8. Measures to track progress of the strategy, once implementation has commenced 9. Some outline modelling of the potential for the programme to improve health and well-being outcomes and/or reduce pressure on other parts of the system e.g. through a reduction in primary and secondary care utilisation
