Provision of the Locality Based Home Care and Support Services for people aged 18 and over who are eligible for Adult Care Services living in Suffolk and NHS Continuing Healthcare (CHC) who are registered with a GP Practice in the Ipswich and East Suffolk Clinical Commissioning Group (IESCCG) and West Suffolk Clinical Commissioning Group (WSCCG) and the Great Yarmouth & Waveney Clinical Commissioning Group. (referred to as the CCG's). Additional information: This opportunity is available in the 'Current Tenders' area of www.suffolksourcing.uk.
