LYME AND CHARMOUTH COMMUNITY SERVICE NHS Dorset Clinical Commissioning Group (CCG) is seeking expressions of interest from suitably qualified and experienced organisations to provide Community Services for Lyme Regis and Charmouth. The Aim of the Community Service in Lyme Regis and Charmouth is to complement and contribute to the ICPCS Model of Care (defined below) by supporting our population who are high intensity users as well as providing a proactive approach to care for people with medium intensity needs. The vision is: * An integrated workforce, with strong focus on partnerships spanning primary, community, secondary, social care and mental health; * A focus on personalisation of care with improvements in population health outcomes and reduced utilisation of acute hospital bed resource. Therefore, the services will be tailored reflecting on the key expected outcomes. Clear rational behind choices of service delivery must demonstrate collaborative working across providers to deliver the key features and functions of the ICPCS model of care and the expected outcomes defined for the locality and our System overall. The aim of the Community Services (Lyme Regis and Charmouth) is to support the delivery of the Integrated Community and Primary Care Service model of care with a focus on the: o Proactive management and rapid response to those most complex patients; o Proactive management of people with long term conditions (LTC). The services provided will include the following: Integrated community services/Intermediate care - Core team to include: community nursing, occupational therapy, physiotherapy, generic nursing/rehab assistants Step up/step down beds Community Physiotherapy Service Expressions of interest must be received no later than 12 midday on 17th December 2018 via https://procontract.due-north.com/Login A Provider briefing session for providers has been scheduled for 1.00pm on 21 December 2018. If you wish to attend please confirm when expressing an interest. There is a maximum of 2 attendees per provider. Additional information: This is a Competitive procurement exercise that is being conducted in accordance with a process based on the open procedure under the Public Contract Regulations 2015. The service to which Procurement relates falls within the Light Touch Regime(LTR) governing procurement of Health, Social, Education and other Service Contracts. Neither the reference to 'open procedure' ITT or the use of the term 'Tender process' nor any other indication shall be taken to mean that the Contracting Authority intends to hold itself bound to any regulations save those applicable to LTR provisions.
