The service will provide accommodation-based support for young people aged 16 - 21 with a range of needs. The support model aims to improve the ability to support young people with a wide range of fluctuating needs in a more flexible way; tapering support based on changing needs. Purpose of the support service is to: * Provide accommodation-based support and an effective transition to adulthood and independence for young people aged 16 - 21. * Ensure that young people have a safe place to live and access to appropriate support. * Improve the physical, emotional, and mental health of young people and ensure that they can access relevant universal and specialist services as and when necessary. * Ensure that young people are prepared for future independent living * Ensure that young people have a good understanding of finances and can manage their own and budget effectively * Enable young people in Wokingham to achieve their optimal potential * Ensure that young people are fully supported to engage in appropriate education, training, or employment
