Notice Description

The City Council and Western Locality of the CCG are developing a model of Wellbeing Hubs across the Western Locality. The main aim of the Hubs will be to transform traditional models of care to improve health and wellbeing outcomes and reduce health inequalities whilst reducing pressure on acute, secondary and primary care settings. The purpose of the market engagement is to consider options for the provision of Information and Advice and Prevention and Community services in the hubs at all tiers. This paper will form the basis for discussion at two market engagement sessions - Friday 6th July and Monday 23rd July.
