IMPORTANT PLEASE READ This RFQ for for 1 machine only STORZ MP100 ULTRA as per attached picture NELFT, BEH and C&I Procurement Shared Service requests offers for the provision of the above goods based on a fixed 12 month offer. Please clearly state your offered product's unit of measure and ensure you accompany your offer with a per item price. Offers for alternative products should be clearly stated, and any relevant specifications and information should accompany those variant offers. No carriage charges should be included or subsequently billed. The Trust does not bind itself to accept any offer, either in whole or in part. Your offer must be issued in accordance with the appropriate NHS Conditions of Contract, a copy of which is available on the Department of Health Website at: http://www.dh.gov.uk/en/publicationsandstatistics/publications/publicationspolicyandguidance/dh_121260 The Trust is currently operating under a policy of Socially Responsible Procurement (SRP) in relation to Corporate Social Responsibility (CSR) and it is expected any supplier who engages with the Trust should also have a demonstrable CSR Policy which outlines their commitment to being ethical and sustainable.
