Community Anticoagulation Service - Bury
Notice Description
Provision of community based anticoagulation monitoring and patient medications management. Reviewing of patients on anticoagulation medication and providing qualified and expert support to switch to alternative forms of medication that avoids patients needing to physically attend blood test clinics. Direct award of a contract to the incumbent provider which has been delivering a local service since June 2018 until present. Additional information: The CCG reserves rights to novate this contract during its term, to a future lead provider of local community services, where the CCG may assess it to be appropriate to achieve such integration.
