Notice Description

Fareham & Gosport CCG, North East Hampshire and Farnham CCG, North Hampshire CCG, South Eastern Hampshire CCG, West Hampshire CCG and Southampton City CCG are considering collaboratively commissioning Tier 3 and Tier 4 Specialist Weight Management Services. Tier 3 Weight Management Service, consisting of a Multi-Disciplinary Team providing an intensive level of input to patients unsuccessful in achieving or maintaining weight loss at Tier 2 with BMI >35 with comorbidities or BMI >40 without. The annual contract value for the services is to be confirmed. The anticipated annual contract value is likely not to exceed PS350,000 (based on current funding for the service). Tier 4 - Specialised Complex Obesity Services (including bariatric surgery) for patients with BMI >45 or consider >30 if diagnosed with Type 2 diabetes in the last 10 years. The annual contract value is to be confirmed. The CCGs are inviting organisations who have an interest in and the capability of delivering either both or one of the Tiers to a provider forum event with a view to: * Discuss service models and develop Service Specification to achieve the best outcomes within the budget * Discuss future options for the service i.e. linking Tier 3 and Tier 4 services The event will be held on: Thursday, 21st January 2016 12.30 - 14.30 Eastleigh, Hampshire Additional information: In order to book a place, you must first register with In-Tend 1. https://in-tendhost.co.uk/soepscommissioning 2.
