Notice Description

North Northamptonshire Council (hereafter referred to as the "Council") and on behalf of West Northamptonshire Council wishes to invite Tender Responses for the supply of services in respect of this requirement. The NHS Health Check programme aims to reduce avoidable premature mortality by early identification and management of cardiovascular risk factors and disease in people aged between 40 and 74 years who have no previous history of CVD disease. In Northamptonshire, 46 in every 100,000 people die before the age of 75 from cardiovascular disease considered preventable, which is similar to the England average (45 per 100,000). An IT solution is required to support the operational process and monitor the performance and effectiveness of the NHS Health Check programme at all levels. The purpose of the IT solution is to enable health check providers to effectively carry out the NHS Health Check programme, for local authority commissioners to effectively manage and quality assure the programme's processes and outcomes and for commissioners to link payment to key requirements of the programme.
