Morecambe Bay CCG is interested in engaging in dialogue with providers of Type 1 and Type 2 Structured Education programmes so that they can help us shape our future provision: Background Structured education is an effective self-management tool to help people diagnosed with Type 1 and Type 2 diabetes understand and manage their life long condition. Structured education programmes are an effective and cost efficient way of improving outcomes and are a key part of diabetes self-management when linked with collaborative care planning, screening and medications. Acting early to prevent complications limits Type 1 and Type 2 diabetes impact on a person's life and saves the NHS money. However despite this evidence highlights that uptake of structured education programmes is shockingly low despite recommendations by NICE guidance and the introduction of QOF referral indicators in primary care. Requirements Due to reasons above, Morecambe Bay CCG are interested in engaging in dialogue with providers of evidence based structured education programmes for Type 1 and Type 2 diabetes that are endorsed and accredited by an appropriate body. If you wish to engage in dialogue please submit your interest in writing with the following information: 1) Name of provider 2) Name of contact 3) Contact details (email and telephone) 4) We are particularly interested in understanding the following: * The models and programmes that you deliver * The successes and outcomes of these * Referral rates onto the programme * Patient take up, retention and attrition 5) How you overcome barriers and use innovative approaches to maximise the following: * Awareness of the programmes * Referrals onto the programmes * Patient retention and attrition * Successful outcomes Please send information to Claire Niebieski - System Population Health Manager at Morecambe Bay CCG by 5pm Friday 27th September and express whether you are happy to engage in further dialogue.
