NHS Arden & Greater East Midlands Commissioning Support Unit (NHS Arden & GEM CSU) on behalf of NHS Southern Derbyshire Clinical Commissioning Group (SDCCG) are inviting Expressions of Interest from suitably qualified providers capable of supporting a scheme focusing on High Intensity Users (HIU) of 999 / NHS111 / A&E, GP contacts and hospital admissions delivering for patients registered within Southern Derbyshire Clinical Commissioning Group. The Commissioners are assessing the markets interest, capabilities and capacity to support the evidence from Rightcare, which clearly illustrates potential for cost savings via the implementation of a scheme focusing on High Intensity Users (HIU) of 999 / NHS111 / A&E, GP contacts and hospital admissions. The aim of the proposed scheme is to reduce HIU activity and, free up front line resources to focus on more clients and reduce costs. The CCG's vision is to utilise a health coaching approach, targeting high users of services and supports the most vulnerable clients within the community to flourish. This HIU service was originally implemented in Blackpool (2013). It is the CCG's intension to develop a scheme lasting approximately 18 month, with a maximum financial envelope of PS78,000. The key aims and objectives of the scheme are to: * Support identification and referral of patients into the new HIU service * The clients receive support from the service which will focus on client needs to identify the 'real' reason for calling 999, A&E or an admission * The scheme will also link into the work around Personal Health Budgets (PHB), and will aim to look at using PHBs to effect a long term change in patient behaviour. * The scheme will engage with EMAS, DCHS and voluntary sector providers looking at meeting the needs of these patients for whom the medical model of care is not effective for. * The service then works to de-escalate the needs by offering immediate access to an appropriate listening support service. Once the needs are met, the patient is discharged with the aim of reconnecting the client back into their community with a purpose. The service will also aim to meet those clients who relapse who can be re-referred into the service. The key outcomes for the top 50 users expected are: * Reduce non elective admissions by 50% by month 4 measured by Secondary User Services data set * Reduce 999 calls by 50% measured by EMAS calls figures and SUS * Reduce A&E attendances down by 50% measured by SUS * Increase provision of Personal Health Budget's by 5% Additional information: The aim of the expression of interest exercise is to: 1. Support the CCGs informed decision making on the potential options on the future commissioning of these services. 2. To help refine the commissioning and procurement approach and; 3. To gauge the level of interest from the market of capable providers with Capacity across Derbyshire to deliver the services illustrated above. This is a market testing exercise and does not commit the CCG to carry out a procurement process. Current services will remain in place during this period in line with existing contracts and service level agreements. Bidders who have expressed an interest in this market testing exercise will have the opportunity to submit a separate response to the Contract Notice as applicable.
