Stroke Recovery Service in Redbridge
Notice Description
NHS NEL CCG are seeking an experienced provider to support Stroke Recovery services in Redbridge and Barking and Dagenham and Havering (BHR). The aim of the service is to enable people living with stroke to make the best possible recovery they can and supporting those around them, so that they can live their life to the full following stroke. The Service will be designed to support stroke survivors, their families and carers in their recovery by providing tailored support after discharge back to their home or community and into the longer term. Support begins following a stroke and continues on a needs led basis for an average of one year. Following discharge from the service we continue to support people within the community via our network of stroke support groups and volunteers. Following discharge from hospital an assessment of need is offered to identify the support needs of the stroke survivor and carer or family. We are looking for a Coordinator function to work with the individual to develop a Recovery Plan identifying the actions needed to reach their desired outcomes. Progress is then reviewed and at each stage their recovery plan and needs will be reviewed. The preferred provider will work with patients and their family/carers to identify individuals' post stroke unmet needs across health, social and emotional care domains in Redbridge (within the financial envelope currently being developed and on a unit cost basis in the other two regions, Barking and Dagenham and Havering). The Core Stroke Recovery Service supports the following outcomes: * Increased access to support networks and reduced social isolation * Identification of safety risks increasing safety and dignity * Increased confidence leading to engagement with meaningful activities * Increased knowledge of stroke risk factors leading to adoption of healthier lifestyle choices * Increased confidence, skills and motivation to self-manage * Increased strategies to support achievement of desired outcomes * Reduced anxiety and emotional distress, increased emotional resilience * Increased access to entitled support, which will aid recovery and carers in their caring role * Increased independence for stroke survivors * Increased feelings of reassurance * Increased physical wellbeing If you would like to express an interest in this opportunity, please complete the attached form and return to nelcsu.procurementnorthants@nhs.net by no later than 12noon on Tuesday 13th July 2021.
