NEL Commissioning Support Unit (NEL CSU) are requesting market feedback, on behalf of NHS Waltham Forest Clinical Commissioning Group to inform the potential procurement for a Wheelchair Service In order to assist the CCGs with testing their assumptions and proposed service requirements, your responses to the questionnaire below will explore the following areas: * The service specification and key performance indicators * Timescales to handover existing services (if required) and mobilise new services The responses to the questionnaire may be reflected in the Service Specification issued at Invitation to Tender. We ask that interested providers submit responses to the following key questions using the questionnaire template provided. Respondents are requested to specifically address the questions rather than attaching extensive technical or promotional information as such information will not be considered. Please complete the template and return via the ProContract messaging facility (https://procontract.due-north.com/Login/) - By 12.00pm, on Tuesday 04 December 2018. The following attachments are available to interested parties to further inform the submission of the questionnaire response. These documents are available for download via the ProContract E-Tendering portal at the link provided above. * This Market Engagement Questionnaire * Draft Service Specification, Activity and Costing information The estimated annual contract value is PS800,000.
