Notice Description

South Yorkshire Integrated Care Board (SY ICB) is seeking to appoint a single provider for the provision of a Social Prescribing Service. The Service will enhance the health and wellbeing of the residents of Rotherham and help decrease health inequalities, by providing a liaison and support service for people at high risk of unplanned urgent and emergency care to access non-medical sources of support in the community. The Contract will be for a term of three years with the opportunity to extend for 2 years subject to mutual agreement between the successful Bidder and the Authority. The maximum term of Contract is consequently five years. The available budget is PS709,661 per annum. This figure includes all aspects of the service to be delivered, any bidder including caveats to this service may be considered as submitting a non-compliant bid.
