Primary Care Provision to Mental Health and Learning Disability In Patient Services
Notice Description
Please apply for the tender via Due North. This service is to ensure that those suffering from severe mental illness and/or a learning disability and are resident within the inpatient units listed below can have easy access to a GP or primary care service when they are not able to maintain contact with their own GP. There are four lots. Suppliers can tender for any or all lots. Lot 1: Stewart House (28 beds) Stewart House provides inpatient rehabilitation for patients with severe mental illness. Patients referred typically experience severe behavioural disturbance with challenging behaviour. The patients at Stewart House may be resident for up to 15 months as part of their rehabilitation programme. Lot 1: Mill Lodge (15 beds) Mill Lodge provides for patients with a diagnosis of Huntingdon's Disease. Patients may be admitted for a period of medical review and to stabilise their condition or for end stage management of the disease and palliative care. Patients referred typically experience severe behavioural disturbance with challenging behaviour. Lot 2: The Willows (38 beds) The Willows provides inpatient rehabilitation for patients with severe and enduring mental illness. Patients may have a history of disengagement with mental and physical health services. They require a longer period of inpatient admission to stabilise their mental health, and support to optimise their level of functioning back in the community. The patients at The Willows may be resident for up to 15 months as part of their rehabilitation programme. Lot 3: Herschel Prins Centre (12 beds) The Herschel Prins Centre is a low secure unit that provides inpatient care for patients with a mental illness including personality disorder, with an offending history and/or history of challenging behaviour. The length of admission can vary, but patients may be admitted for up to 24 months as part of their rehabilitation care pathway. Lot 4: Agnes Unit (12 beds) The Agnes unit is a 20 bedded unit, (currently commissioned for 12 LLR beds), divided into 5 pods of 4 beds each. The unit caters for adults with a learning disability who have mental health problems, pervasive developmental disorders, cognitive problems, offending history and challenging behaviour. It provides longer stay rehabilitation as step down into the community, as well as assessment and treatment for acute presentations. The provider for Lot 4 may, on occasion, be required to visit a service user at one of the Learning Disability Short Break units based on the Glenfield Hospital site (This would be within the allocated session for the Agnes Unit). The LD Short Break Units provide respite care for individuals with a learning disability and health needs, assessed through a continuing care assessment. The units provide planned stays throughout the year to individuals who live at home with families, in order to facilitate a break for the carers and also provide a therapeutic opportunity for service users.
