Neighbourhood Homecare Key Provider Service
Notice Description
Regulated homecare plays a key role in the Councils homecare offer, forming part of a network of support for People to choose from. Alongside regulated homecare, the Council have developed the market to include community led support through increased volunteer activity and community micro enterprise provision , ensuring we are offering choice to People and meeting our responsibilities under the Care Act in supporting and developing a diverse market. The Neighbourhood model will seek to identify regulated providers who are ambitious to work in partnership with the Council, focussing on supporting citizens to achieve their 'best life'. Successful asset and outcomes based provision requires a transformation period with all stakeholders working in partnership to ensure provision that is deliverable and clear in ambition and intention. Changes in culture and practice can only be achieved successfully where people who receive services, providers, social work practitioners and commissioners all work in partnership - the Key Partner Provider ( KPP) will be instrumental in making these changes in relation to provision on homecare. Each neighbourhood KPP will head up a two tier system - the KPP underpinned by a Framework. KPP will benefit from market leading profiles as the Councils 'first point of contact' and key development partners in our journey in enabling, asset based care. KPI are measured by successful partnership working as much as volume of business, enabling providers to innovate in care delivery, with KPP expected to develop and maintain relationships with Reablement and Intermediate Care, Primary Health and community and voluntary sector system partners.
