The delivery of a new AHP ACP credential that describes the capabilities to: Support people with LTCs to live well with their condition, remain active and independent and reduce reliance on secondary care. Lead the transformation of community services to deliver high quality integrated and sustainable community rehabilitation. Build and lead a wider rehabilitation workforce (SW and ex professionals).
