Notice Description

Southend and Castle Point & Rochford CCGs ran a procurement process commencing on 9th January 2019 to commission a framework of multiple providers with the appropriate capacity and capabilities from which they can spot purchase high quality services for patients requiring Intermediate Care (Bedded Provision) (ICB). Intermediate Care services will operate as part of a wider Discharge to Assess approach being implemented across South East Essex, which requires a consistent approach to both the provision of rehabilitation services and proactive planning for the next stage of the persons recovery. The expectation is that this service will work with Provider partners as part of Intermediate Care community-based services, to provide nursing and therapeutic input for individuals who require 24 hour supervision during their rehabilitation and reablement programme. The primary aim of the service is to deliver intense reablement / rehabilitation to maximise a person's capacity for independent living, increasing the number of people able to remain in their own homes, and reduce the number of people permanently admitted to long term care and/or requiring long term adult social care / NHS continuing Healthcare funding. The evaluation process has concluded with one provider being awarded a place on the framework.
