Notice Description

Kent Community Health NHS Foundation Trust are seeking a provider of a Kent Health Improvement Service IT system which will record all outcomes from clients wanting to access a service to support a behaviour change. This will include recording of an initial assessment, to booking an appointment and navigating the client to a service that is accessible to them, recording of further assessments once at the service entry point, recording of follow up appointments and to measure long term behaviour change outputs to have a function to record outcomes at 6 and 12 months after sign off from the service. The vision for this system is to offer an integrated health improvement service whereby three of the services Stop Smoking, Health Trainers and Healthy Weight integrate. NHS Health Checks is a separate programme however; it must work closely with the integrated team to help measure outcomes from a patient's journey. This contract will be for a fixed initial term of 2.5 years (30 months) with potential option to extend up to a maximum of 5 years ( This agreement will be set out for 3 years + 1 year + 1 year with an option to terminate after 2.5 years).
