Notice Description

Patient Discharge Service (PDS) is a subset of Patient Transport Service (PTS) and renamed Discharge Patient Transport Services (DPTS). It caters for patients who without effective, safe transport could have further problems due to difficulties returning home or to a place of care. Journeys will be required both with advance notice and 'on the day'. This can and should encompass a wide range of vehicle types and levels of care consistent with the patients' medical needs. The overarching principle of DPTS is that patients who are eligible for transport will receive safe, timely and comfortable transport, without detriment to their medical condition. The landscape of health care in South Yorkshire is changing and this will continue to develop with care moving away from traditional secondary care settings into more primary and community settings. All changes will be made in consultation with the transport provider, the community and other stakeholders DPTS in South Yorkshire is commissioned for patients, for which the local Commissioner has responsibility, to ensure that patients are discharged from hospital in a safe and timely manner. The DPTS provider must be ready to adapt and innovate in line with health care provision. Journeys from inpatient care to other inpatient care facilities such as community hospitals and hospices are considered as Inter-Site transfers. The current guidance underpinning this service specification is that issued by the Department of Health in 2007. In the event of changes to National Policy Guidance, General Condition 13 in the NHS Standard Contract terms and conditions will apply. The service specification has been developed by NHS Barnsley CCG on behalf of NHS Rotherham and NHS Doncaster CCG to provide a framework for an out of hours (OOH) Discharge Patient Transport Service in Barnsley and Doncaster along with a full discharge service in Rotherham. Aims and Objectives of the Service. To provide three (3) separate, dedicated discharge services that operate out of three different acute hospitals: Barnsley Hospital NHS Foundation Trust The Rotherham Hospital NHS Trust Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust. The Contract will be for the period 30 November 2019 - 31 August 2022 with option to extend for a further 1 year.
