Notice Description

The Council is commissioning the Tower Hamlets Healthy Communities Programme in order to improve population health and wellbeing and reduce local health inequalities. The new programme will address health and wellbeing issues across the whole life course, from infancy to old age. The programme will predominantly support the 'Communities Driving Change' priority in the Tower Hamlets Health and Wellbeing Strategy 2017-2020, however we anticipate that it will also impact on each of the other high-level areas that have been prioritised within the Strategy: Creating a Healthier Place; Employment and Health; Children's Weight and Nutrition and Developing an Integrated System . We want to build on the great work that has been achieved through previously commissioned programmes, whilst moving away from a model of service delivery to a model of community empowerment. In line with the Council's commitment to co-production we want the programme to be shaped by local community priorities as well as by the expertise of local providers. We are therefore issuing an outcomes-based specification and asking providers to identify the most effective way of working with local people to address their priorities, as well as achieving the outcomes set out in the Health and Wellbeing Strategy. The programme will be split into 3 phases however we are procuring the three phases together, so are looking for Providers who can effectively deliver across all three phases. Phase 1 - Scoping (3 months): - identifying 2 or 3 geographical or topical areas of focus - identifying the key stakeholders within each area of focus Phase 2 - Engagement and development (3 months): - engaging and understanding the local community and relevant stakeholders - identifying local assets, building on rather than duplicating what exists - co-producing local outcomes frameworks and delivery plans for each area Phase 3 - Delivery, reflection and review of the delivery plan (30 months) - delivery of the plan produced in phase 2 - monitoring of progress against the outcomes agreed in phase 2 - review of progress and refresh of the delivery plan We are procuring the programme in four locality-based lots, with providers being welcome to bid for one or more of the lots. We welcome consortia bids. The anticipated contract start date is: Monday 2nd October 2017 The estimated value of the contract is: Lot 1 PS200 000 per annum Lot 2 PS200 000 per annum Lot 3 PS200 000 per annum Lot 4 PS200 000 per annum For a period of: 3 + 2 years Organisations wishing to express their interest are requested to register on the London Tenders Portal via the link below, where the tender documentation will be available to download.
