Community Crisis Support Service in Nottinghamshire

Notice Description

The crisis support service will provide intense and focused health and social care (including personal care) to assist people through a worsening health crisis to remain living in their own home and maintain independent living skills. The key elements are a rapid response to the individual's home, a thorough assessment and an integrated care package allowing for short term results and a long term solution. Benefits of the service will be: Patients and carers will be supported to maintain their independence as much as possible and regain health status following an exacerbation and/or health/social care crisis. Unnecessary hospital admissions may be avoided as the service will provide alternative support Improved quality of life for patients and carers through certainty of support, quality of service, integrated care programmes, enabling the patient to remain independent in their home, providing additional carer support The service supports GP Practices and Community nursing/therapy services by offering a timely and intensive support service. This enables the clinicians to work efficiently with a crisis support service to enable an integrated team to support patients to remain at home. The appropriate skill set is accessed to deliver the appropriate care in the most appropriate care setting.
