Notice Description

This Invitation to Quote is for GP/ANP Cover for Marrow House. The overall aim of the practice and GP service is to provide 5-day, dedicated GP/ANP support to the 21 beds at Marrow House, Longton for older people with mental health needs - primarily dementia by: * Providing a holistic assessment and report of the patient within 24 hours of admission to Marrow House - 5 days per week. (Where a patient is admitted on a Friday afternoon/ Saturday or Sunday the assessment needs to take place on a Monday morning). * The report will be shared with staff prior to the MDT meetings. The GP/ANP will be responsible for carrying out appropriate actions following the MDT e.g., medication reviews, input into the crisis care planning and EOL if appropriate. * Where necessary the patient will need to be temporarily registered with the GP's practice whilst undergoing assessment at Marrow House. * The GP will be responsible for reviewing and updating/initiating ReSPECT forms where service users wish to complete the document. * The practice will be responsible for providing cover during any leave. The provider must be a local primary care practice within a 10 mile radius of Marrow House, to provide GP dementia cover to the 21 beds where the length of stay for patients is approximately 2 - 6 weeks. The service will be available for 5 days per week Monday to Friday during practice opening times. The NHS Standard Contract will apply. The contract will be for a period of 24 months with a start date of 1st July 2023. The Commissioner has set an indicative affordability threshold of PS30,000 per annum. This value will be inclusive of any 2.5% allowable CQUIN payment. This is an indicative threshold set to reflect the expected costs of providing the service specification and Bidders are encouraged to avoid exceeding the threshold. The ICB reserve the right to reject quotes that are above the affordability envelope.
