Notice Description

West Sussex County Council Reablement Services Introduction The provision of reablement services across West Sussex to support customers following a change in ability, possibly caused by a medical condition. The intention of the reablement service will be to re-skill the customer to enable them to retain or regain their independence and to prevent or reduce the need for longer term support. It can be provided to a customer following a community based assessment or upon their discharge from hospital. Reablement is recognised as an important tool in helping the Council support an ageing population, with increasingly complex needs, in West Sussex. Typically reablement services are provided to people being discharged from hospital or entering the care system following a crisis. It involves specialist care workers providing support to customers in tandem with occupational therapists, health professionals and other providers of services and support. Our current pathways can work in isolation from the health provision of 'Intermediate Care'. There is therefore an exciting opportunity, through joint working and coproduction with the council and health partners, to further align the Councils reablement services with local health responsive services, contributing to future joint service developments and design. The Council is seeking to obtain the views of providers on the opportunities for the delivery of innovative reablement services to obtain the best outcomes for customers and how best this can be aligned and delivered in partnership with health services. Next Steps The Council are looking for providers to participate and have an input into an initial market consultation exercise. The outcome of the discussions will enable the Council to incorporate the information into our requirements for the contract. The event will take place on Thursday 16th February 2017 at County Hall Chichester. You will be allocated a 1-1 appointment with our team. Parking is not available on the County Hall Campus however there are numerous car parks with a 10/15 minute walk of the Campus. To participate in the market consultation event please express an interest in this project and send a message via the correspondence facility on the portal no later than 4pm Monday 6th February 2017. Once you have confirmed that you wish to participate a time slot will be allocated to you and further information will be provided. If you do have any preference to a particular time, please send a message via this project and we will do our best to accommodate your request.
