Notice Description

South Cheshire Clinical Commissioning Group and Vale Royal Clinical Commissioning Group are seeking a provider from suitably qualified and experienced organisations to provide new self-management education modules, toolkits and care pathways for people with, or at risk of, osteoarthritis of the hip and knee to people in their local communities. The service will be providing lifestyle coaching, exercises, shared decision making discussions and run by allied Health Professionals (Physios) and will also be providing pain management advice. The service will provide lifestyle coaching to include weight management - healthy eating and the effects of reducing weight or management of weight to support with joint pain, rehab and rehab post-surgery. Sessions will need to run Monday to Friday providing group sessions for up to 6 patients for 1 1/2 hour sessions and will need to be held across the cluster areas of the CCG. Each patient is likely to attend approx. 3 sessions, each of them being a month between each appointment. * It is the intention of the Commissioners to enter into a 2 year contract with a possible extension option of up to 12 months. This Tender is subject to the Light-Touch Regime as set out in Schedule 3 of the Public Contracts Regulations 2015 (Annex A). Additional information: If and when this requirement is offered to tender, this will be done via electronic means using the internet. Midlands and Lancashire CSU is conducting this procurement exercise as a central purchasing body for another NHS body with whom the successful bidder(s) will enter into contracts for the supply of the services. Any other public sector body wishing to access the contract may do so only with permission from the contracting NHS body. The contract conditions will be set out in the Invitation to Tender.
