Primary Care Networks were established as part of NHSEI's long term plan to build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home The Public Participation Team receives funding from the PCN Development Team to support the development of PCNs in how they work in partnership with people and communities.
