NHS Medway Clinical Commissioning Group and Medway Council (the Contracting Authorities) are seeking to procure a high quality, effective and outcome focused service that will improve health and wellbeing outcomes for local people and offer good value for money. Commissioners want to pilot a new service model to support individuals living with long term health conditions to improve their skills, knowledge and confidence to manage issues impacting on their long term condition/s more effectively. The pilot will take place initially in one Local Care Team locality (Rochester). Quotation documents including the service specification are accessible on the Kent Business Portal (www.kentbusinessportal.org.uk). Interested Providers would need to register to set up an account for their organisation on the Portal in order to express an interest and view the documents. Registration is free.
